As one of the first independent service coordination agencies in the State of Illinois, we have a wealth of information for you. Please choose from one of the options above to learn more about PACT and the services it provides. In particular, take note of the opportunity to participate in our online Discussion Forums.

We can be contacted by a variety of methods. The column at the right shows different methods to use in contacting us. We maintain a 24-hour answering service for emergency situations.

There are no charges for any of our services except for those in our Life Management Services® program.

If you have questions regarding any of our programs or would like to initiate the process to receive services please call 630-960-9700, ext. 127 for our Child and Family Connections Intake coordinator, ext. 128 for our Community Services Intake Coordinator, and ext. 103 for our Life Management Services® Intake Coordinator.
