Community Case Coordination
What The process of obtaining community services for persons with developmental disabilities can be a difficult maze to navigate.  PACT's Community Case Coordination program provides the central connection between the individual and community services.  Our service coordinators will assist persons who are residents of DuPage County, and who are seeking residential, day programs, and vocational services.

 

Services Services Include:
  • Intake
  • Gather information and assessments needed to determine eligibility for services and for referral to service providers
  • Identify choices available to the person with disabilities
  • Service Planning & Reviews
  • Initiate referral process with the goal of acceptance into programming
  • Secure funding from relevant state departments for residential and other services
  • Coordinate visits and meetings with potential providers
  • Complete all required funding and authorization forms, or assure their completion by the appropriate party
  • Education for individuals and community
  • Crisis Intervention
Fees There are no fees for any of the services that Community Case Coordination provides.
