Pre-Admission Screening (PAS)
What For persons and families seeking community residential and day services Pre-Admission Screening is the first step.  PACT provides this service to residents of DuPage County who have a developmental disability.

This program will complete an eligibility screening to determine if a person meets the criteria for state funded services.  We will also identify which services a person is eligible for, and link them to available programs.

 
Services  Services include:
  • Gather information and assessments needed to determine eligibility for services and for referral to services providers
  • Complete state-mandated screening
  • Identify choices available to the person with disabilities
  • Secure funding from relevant state departments for residential and other services
  • Coordinate visits and meetings with potential providers
  • Complete all required funding and authorization forms, or assure their completion by the appropriate party

 
Fees There are no fees for any of PACT services in this program.
